The Department of Primary Industries and Regional Development (DPIRD) (we, us) is collecting your personal information (your information) through this form.

We are collecting your information so you can notify DPIRD of the commencement of works, active works, or completion of works carried out under a permit issued under Part 7 of the Fisheries Management Act 1994, and so DPIRD can monitor compliance with the conditions of that permit.

We will not share your information with anyone else unless you give your consent, or the law requires or allows us to do so.
Are you required by law to give us your information?
Yes. You are required by law to give us your information as a condition of a permit issued under Part 7 of the Fisheries Management Act 1994.
If you choose not to give it, you may be in breach of a permit condition, which may result in compliance action, including penalties, and may affect your ability to lawfully carry out the permitted works.
